The purpose of this questionnaire is to help us understand your doctrinal position.

Obviously we expect you to be in full and unreserved agreement with the doctrines of your Church, but as you know there are many issues and “winds
of doctrine” prevalent among independent Baptist churches today. Many of which are harmful and destructive to the cause of Christ.

This Church believes that it should only help support evangelists (“missionaries”) sent by other churches of like faith and practice who are compatible
with our doctrines, aims and methods.

Please answer all questions completely. After you submit the questionnaire we will contact you concerning whether or not we can schedule you to
present your work to our church.
